The total is $4,314 when paid within one year. Otherwise, the plan carries 29.99% interest for 36 months, resulting in a total payment of $6,456.

The practice states that approval is available except in cases involving prior money laundering, an open bankruptcy, or an HFD delinquency.

Insurance Payment Sent Directly to the Practice

Direct Assignment

Patients with qualifying orthodontic insurance benefits may arrange to have the insurance payment sent directly to Bagnall Family Dentistry.

The down payment is calculated by subtracting the approved insurance benefit from the $3,579 treatment total. A minimum down payment of $1,000 is required.

This option does not apply to United Concordia or COBRA plans.
